Collaboration From the Start
We begin with an initial conversation — by phone or virtually — with you, your family, advocate, or any healthcare professional involved in your care. This helps us understand your needs and explain how our services can be tailored to support your clinical, personal, and social wellbeing. If requested, we can provide a plan based on your specific requirements.
Comprehensive Needs Assessment
We then offer a free home visit with a Registered Nurse to explore your care needs in detail and understand your home environment. During this visit, we conduct a general risk assessment to ensure that your care plan is safe, practical, and tailored to your circumstances. Following this assessment, we provide a detailed proposal and final plan, outlining exactly how your bespoke care package will be delivered.
Your Personalised Care Plan
Once you are ready to proceed, we finalise your personal care plan, detailing the care you will receive, how it will be delivered, and most importantly, your personal preferences. Your plan is fully agreed with you, your family, advocate, or healthcare professionals before we bring together your dedicated care team.
Building Your Care Team
Our specialist in-house care team carefully matches our skilled carers to your individual needs. We can make sure the staff that will be looking after you are trained in your specific care requirements. Once your team is selected, they will be introduced to you and your care plan and begin providing care at a date and time that suits you.
Training and Competence
All our Healthcare Assistants and Support Workers undergo a comprehensive induction, including meet-and-greet sessions, shadowing, and specialist training to ensure they have the skills needed to deliver your care safely and effectively. Our Registered Nurses provide oversight and in-home training to ensure competence in all clinical tasks. This ensures high-quality, professional care at every stage.
Ongoing Support and Quality Assurance
Our commitment doesn’t end once your care begins. We regularly check in with you to gather feedback and ensure your care continues to meet your expectations. Our team also conducts quality audits and continuous monitoring so that you consistently receive the highest standard of service from West Park Care.
